Guidelines to Providers for Completion of VR-370 Monthly Service

Information/Tips

As soon as possible, please provide the VESID District Office(s) with your contact information.  This person should be the individual responsible for the completion of the VR-370 Report.  Include the individual’s name, phone number (including extension) and e-mail address.  If this information changes, notify your District Office liaison.

Contact your VESID liaison for the following:

  1. Clarification of the provider guidelines.
  2. Questions regarding the completion of the VR-370 Report.
  3. If you do not receive your VR-370 Report within the first two weeks of the month.
  4. If you know you will be late mailing your completed VR-370 Report to the District Office.

Please remember that your payment for consumer services provided is based on:

  • The timely submission of the VR-370 Report to the District Office.
  • The timeliness of VESID staff data entry.
  • Timely submission of consumers’ progress, attendance and/or evaluation reports, etc. as required for the service.
