Guidelines to Providers for Completion of VR-370 Monthly Service
Rules for Successful Completion of Your VR-370 Report
- Review the previous month’s Report for comments (if any) written by VESID staff and follow-through appropriately.
- Be aware that the start date for the delivery of service for each consumer cannot precede the start date on the authorization.
- Because this is a monthly report, only those services provided within that month can be entered on the report. Use the last page to report any units from previous months.
- The end date of service cannot be after the authorized end date, unless prior approval has been requested from the VESID counselor.
- There may be two (2) authorizations for the same consumer. Please make sure to report the delivered units of service separately on each authorization. Do not combine the two authorizations.
- There may be two (2) lines of service authorized for the same consumer. Please make sure you report the units on the correct line of service.
- Do not report more units than have been authorized. Requests for additional units of service must be made directly to the VESID counselor within that particular District Office.
- If the “Units Available” does not reflect deductions from the previous month’s report, do not re-enter units of service from that month on the current month’s report.
- Make sure you sign and date the last page of the VR-370 Report for each District Office before mailing. Unsigned reports will be returned to you and will most likely result in a delay in your payment.
- Make sure you mail the completed report to the correct District Office. The District Office is listed in the top, left-hand corner of the VR-370 Report.
- The report is to be mailed to the District Office even if there are no units to report for that particular month. This is to ensure accuracy in tracking for reconciliation purposes.
- When requesting cancellation of units, please make sure that the cancellation “mark” is clearly noticeable. You may also simply write, “Cancel” in the space.
