Guidelines to Providers for Completion of VR-370 Monthly Service

Questions and Answers

  • Q: What is the VR-370 Report?
    • A: The VR-370 Report is your usage report for services provided each consumer listed on the report in a given month.  The report is an alphabetic listing of all consumers who currently have open and valid authorizations with VESID, including start date, end date and units of service available for reporting purposes.
  • Q: What is the purpose of the VR-370 Report?
    • A:  The purpose is two-fold:
      • To report units of service provided each consumer during the month.
      • To serve as a “communication tool” between VESID and the provider.
  • Q: How often will I get this report?
    • A:  The VR-370 Report will be sent to you at the beginning of each month.  You should receive the report within the first two weeks of the month.  If you do not, contact your District Office liaison as soon as possible.  He/she will contact Central Office directly and get back to you with their response.
  • Q: What do I enter on the VR-370 Report?
    • A:  The monthly units of service provided to each consumer during that given month.  Fill in the start date, end date and units for each consumer who received services and who have valid VESID authorizations.
  • Q:  What if I have unreported service units that were delivered prior to the current month?
    • A:  You must report these units on the last page of the VR-370 report.
  • Q:   Where do I mail the report after I have finished entering the necessary information?
    • A:   Mail the report to the District Office listed in the top, left-hand corner of the report.  Be careful when mailing in your completed report.  It may include consumers from more than one District Office.  Staff from one District Office cannot enter information on consumers from a different District Office.  Therefore, to avoid delays in data entry, be sure you are mailing the report to the correct District Office.
  • Q:  How will I know that the VR-370 Report has been received and processed?
    • A:  You should receive a photocopy of the report back from the District Office after all valid units have been entered.  If you do not receive one, contact your District Office liaison.  Your District Office liaison will notify you if the report has not been received.  This will occur the last week of the month.
