There is a new VR 370 reporting package which is being distributed to
all VESID UCS Contractors. Included in the package is:
* Cover letter to Provider Agency Liaisons.
* Guidelines to Providers for Completion of
VR- 370 Monthly Service Report ( Issued 8/1/05)
* Sample Packet of completed VR 370 reports.
* AC 92 Standard Voucher with
instructions (pdf version)
* AC 93 Continuation Sheet.

Liaisons
This is the first time there have been written guidelines compiled in
such manner for VESID Staff and UCS Providers and represents a
significant milestone. We are aware that there will be issues needing
clarification and plan on updating these in the coming months, if
necessary, but at least annually.
