Transition for Students

Print This List To Keep on Track

(Check off each box as you complete each step.)

Meet with your TLC, Transition Linkage Coordinator, at your school to find out if you are eligible. You can find this person by asking your teacher or the assistant principal.  

Bring your Social Security Card to your TLC to copy for your VESID application. 

Fill out all of the paperwork and have your parent/guardian sign the release and VESID application.  If you are 18 or older you may sign the release and application yourself. 

Attend all VESID meetings at your school or at the VESID office.  

Expect a letter from your VRC, Vocational Rehabilitation Counselor, at VESID informing you that you are eligible for services.  This letter may include a meeting date and time.  Be sure to attend this meeting and if you cannot, you must call to reschedule.

Bring your Eligibility Letter to your TLC and discuss options for your future.

Meet individually with your VRC to discuss your future plans.  Don't know what your plans are?  VESID can help you figure that out.

If you have a problem, call your VESID counselor (VRC) right away.  They are there to help you work things out.
