PROGRAM WAIVER
ANNUAL EVALUATION REPORT
Report Format
The waiver process includes a provision for an Annual
Evaluation Report. Each program must submit an annual report within two months
of the end of each school year to the State Education Department. The report may
be submitted through the program director, the Superintendent of Schools or the
Chancellor of the New York City Department of Education or his designee.
- Program Description Summary – Provide a brief summary which describes
the program that was implemented and identifies the Regulation(s) waived along
with the number of students impacted by the waiver. The intended objectives
(i.e., expected improvements or results) of this program are to be concisely
and specifically stated. Any changes from the original application will be
noted.
- Evaluation Design – Describe the method and provide copies of
assessment tools that were used to evaluate, monitor and document the progress
of the program. Indicates the criteria used to measure success.
- Evaluation Results – Document the specific results achieved and the
overall effectiveness of the waiver. Also, identify related or expected
outcomes, such as new methods of operation, uses of new technology, cost
savings, or linkages with higher education, community institutions and
business. Where data on student performance is collected, summary tables of
results are to be provided.
- Report Summary – Indicate any request for modification or extension
that may be necessary to improve or to continue the program. Provide a clear
rationale for any requested changes.
