APPLICATION FOR NEW 2003 JULY/AUGUST
EXTENDED SCHOOL YEAR PROGRAM AND SERVICE (S)
Please complete the following for each NEW extended school year program/service(s):
Form J/A 03: Application for Approval of July/August Program/Service(s)
Your agency/school may apply for any of the following program models; however, there can only be one program model per application:
Also included in this packet are two separate and different financial forms. These are necessary for the development of the rate for July/August program/service(s) approved under Section 4408 of the New York State Education law. Your agency/school will need to complete one of these financial forms depending on the program model listed above.
Complete the Special Education Services Budget if your school is applying for approval of any of the following program/service(s): Home/Hospital Instruction (9022), Specialized Instruction (9029), or Specialized Instruction with Related Services (9025).
Complete the Related Services Only Cost Center Report if the program/service(s) is Related Services Only (9015).
Full-Day (9000) or Half-Day (9010) Special Class Program does not need to complete either of these financial forms because rates for this kind of program model are determined by a Regional Weighted Average (RWA).
Applications should be submitted by April 25, 2003. Submission of applications after that date will not be assured review prior to the initiation of the July/August program/service (s).
