| SUBJECT: | Actual September 2001 - June 2002 Operational Dates of Services for School-Age Programs in Approved In-State and Out-of-State Private Schools and Special Act School Districts for Students with Disabilities, Ages 5-21 |
Please be informed that reimbursement for the costs of educational services for school-aged students with disabilities, ages 5-21, who are placed pursuant to Section 4402 or 4407 of the Education Law for the 2001-2002 school year, must be requested through the System to Track and Account for Children (STAC). It is necessary that the dates of service for these students be provided on STAC forms. These dates will be compared to the dates of service for the approved private school or special act school district program in order to determine a full-time equivalent to be used in computing State Aid for each student.
Attached is a list of private schools and special act school districts approved by the New York State Education Department to serve students with disabilities for the 2001-2002 school year under Section 4402 or 4407 of the Education Law. Included are the beginning and ending educational service dates approved for each program. Please refer to these dates when requesting approval for reimbursement through the STAC processing system for the costs of educational services provided to students with disabilities placed in these programs.
Please be aware that not all of the approved private schools or special act school districts have responded to our request for actual operational dates. If an approved private school or special act school district is not listed, please contact them directly to identify its beginning and ending education service dates.
Students with disabilities who are placed residentially must have exact dates for services for education indicated on their STAC forms. However, maintenance dates for full-time placements must be indicated as 9/1/01 - 6/30/02. For less than full-time placements, use the exact dates the student is in residence.
Please share this memo with all persons within your district/BOCES who are responsible for completing the STAC forms used to request funding approval for services to students with disabilities.
