Section 602(34)
The definition of transition is changed to the following:
"results-oriented process that is focused on improving the academic and functional achievement …to facilitate movement from school to post-school activities …."
Section 614(c)
IDEA 2004 consolidates transition requirement to begin "not later than the first IEP to be in effect when the child is 16" with annual updates thereafter.
The IEP of such students must include post-secondary goals that are based upon appropriate transition assessments related to training, education, employment and, where appropriate, independent living skills
The transition services on the IEP must include courses of study.
Section 614(c)(5)(B)(ii)
When a student's eligibility for special education services ends (graduation or aging out), the LEA is required to provide the student with a summary of performance report of the student's academic achievement and functional performance, which must include recommendations on how to assist student to meet postsecondary goals.
