List of Outstanding Certification and Assurance Forms

According to our records, below is a list of school districts (sorted by Regional Information Centers) that have not submitted the required Certification and Assurance form to certify their December 3, 2007 Special Education Snapshot data. Please follow the steps below to record the date of data verification you are certifying and print and mail your Certification and Assurance form:

• Log into the PD System at: http://pd.nysed.gov/;
• Look under 2007-2008, click on the link that says “View Ink Signature Certification and Assurance Form for 2007-08”;
• Select the date on which you saw your data as being complete and accurate. Once you do this you are locking in your data; and
• After clicking the submit button you will be taken to the Certification and Assurance form. Please print this out, have it signed, and mail in your original to the address noted above.

Please submit your Certification and Assurance Form by May 15, 2008.  Thank you for your cooperation.
