This link is available for all schools, districts and agencies that submit students with disabilities data on the PD forms to update the contact person information, including an e-mail address. SEDCAR will communicate with schools’ contact persons via e-mail. Please ensure your school’s contact person information is current.

REQUIRED PD SUBMISSIONS FOR THE 2007-08 SCHOOL YEAR

The PD report forms that are due during the 2007-08 school year are now available for your use at this site. The due dates are identified next to each form. The cover memo accompanying each PD form includes a summary of changes compared to the previous year’s form.

Please note, the State Education Department will not be mailing these forms. The forms may be printed from this site. The Word and PDF versions of the forms are best suited for printing. The data for these forms (PD-6 and PD-8) must be submitted electronically through the PD web-based data entry system at http://pd.nysed.gov or through the File Transfer Protocol (FTP) process. Paper copies of these reports will not be accepted.

Note: Schools are requested to look under the appropriate headings below to identify which PD forms they are required to submit.
