Transition
planning and services are required by Federal and State law and regulation.
It is a process that prepares students with disabilities to live, learn,
and earn in the community as adults.
Transition services
are a coordinated set of activities for secondary students with disabilities,
designed to achieve specific outcomes, which promote movement from
school to post-school activities. The coordination is carried out
by the school district, with the active participation of the student,
his or her family and appropriate community service agencies, including
VESID.
There are seven regional Transition
Coordination Sites to assist with the implementation of transition planning and services
statewide. The Sites can help to coordinate existing resources and provide information,
training and technical assistance on transition related topics for those involved in the
planning and provision of transition programs and services. More information on the Sites
