Vocational rehabilitation is a process that involves an
application for services, eligibility determination, vocational counseling and the
development of an Individual Plan for Employment (IPE) to provide services leading to
employment.
There are various ways you can access services in the
Hauppauge VESID District Office and its Riverhead satellite office. You can be referred by
your treating physician or treatment center, apply yourself, or if you are a high school
student, your guidance counselor can refer you.
You can call the Hauppauge office at 952-6005
and make an
orientation appointment for both the Hauppauge and Riverhead satellite offices. Our
orientation schedules are Tuesday and Wednesday in Hauppauge and Monday in Riverhead. An
appointment is required since space is limited! When you make an appointment, medical
forms will be mailed to your home to give to your physician to be filled out. If you have
medical reports from your physician, bring them to the orientation sessionit may
help speed up the application process.
During the group orientation we will give you an overview
of the VESID program which will cover topics such as:
- The mission of VESID
- How one becomes eligible for services
- VESIDs goal of employment for every consumer
- Medical assessment
- Confidential Health Questionnaire
- Types of services available
- Economic need
- Consumer rights and responsibilities
- The Client Assistant Project (CAP)
- The appeal process
- Next steps in the rehabilitation process
After the group session of the orientation is completed, if
you are interested in following through with your application, you will meet individually
with a VESID staff member. During this interview you may want to ask questions about your
specific situation and how VESID may be able to assist you. We will want to discuss your
disability and how is affects your ability to work. This information is needed to
determine whether you will be eligible for VESID services.
If you do not have medical reports with you, or if
additional information is needed, we will give you the necessary medical forms you will
need to have filled out by your doctor. Once we have the necessary information, you will
be notified by letter what the next step in the rehabilitation process is.
If you cannot get a report which describes your disability,
VESID may be able to arrange for evaluations needed to determine eligibility, but this
will definitely take longer. Once you are determined to be eligible for VESID services,
you will be assigned to a vocational rehabilitation counselor. You will receive an
appointment letter arranging an appointment with your counselor. It is very important for
you to arrive at the correct date and time so you and your counselor can start working
together to plan your vocational future.
