Intake
The Bronx District Office has a liaison relationship
with many treatment programs and schools and various community service
providers. If you are a person with a physical, mental or learning
disability, and want to enter or return to the world of work, you are
invited to visit our office, and learn more about the array of services
offered by VESID. For general information call (718) 931-3500.
In the Bronx, experienced vocational rehabilitation
counseling staff conduct two intake orientation sessions every Monday at
9:00 a.m. and 1:00 p.m. You may call the office at (718) 931-3500 and
schedule an appointment to participate in one of the sessions. It is
important that you bring a pen, sign in when you arrive, and provide
your social security number.
You will see a video explaining VESID services. (Note:
You will not be admitted into the orientation sessions, once the
orientation video begins.) A counseling staff person will meet
with you briefly to discuss your disability. He/she will provide you
with a VESID application packet and specific medical forms to take to
your doctor, therapist, counselor, or social worker to complete.
A representative of the Bronx Independent Living
Center is usually available to schedule appointments for people who
receive SSI/SSDI benefits, and want to discuss how returning to work
will effect their benefits, and for others who may have issues with
housing, and social service entitlements and legal concerns
Intake/Eligibility
Determination
After your referral packet is completed, please
call (718) 931-8394 between 9:00 a.m. and 12 noon Monday through Friday,
and ask for an intake/eligibility determination appointment or
select the telephone option for intake/eligibility. You will be given a
date and time to return to the office, and meet with an eligibility team
vocational rehabilitation counselor. Please bring the completed
referral packet, any award letters, and other pertinent information to
your interview. At that interview, your eligibility will be
determined, and your case will be assigned to a vocational
rehabilitation counselor for guidance and counseling. The vocational
rehabilitation counselor will follow-up by sending you an appointment
letter to visit him/her at the office to begin the process of developing
your individualized plan of employment.
